         Bailiwick Social Prescribing Link Worker

                & Scheme Administrator

Do you want to help people improve their health and

well being through personalised links with social and

community groups, physical and creative activities,

volunteering and support from statutory services?

          Link worker: 28 hrs per week

                   Hours negotiable

working 1:1 with a caseload of adults you will use

motivational approaches to find out 'what matters to 

them' and help people to access community-based

support to improve their physical, mental and

emotional health and wellbeing.

          Administrator: 21 hrs per week

                Hours negotiable

You will provide administrative support to the Bailiwick

Social Prescribing team, including onboarding clients

and maintaining records, taking calls, arranging

meetings and appointments and taking minutes.
